Insert mode vs Overwrite mode in DMW

If you are in Insert mode, then rewinding and recording will split the dictation and put a new chunk in the middle. This often means that you end up with a dictation with a bunch of unwanted bits at the end. Overwrite mode is more like a traditional tape recorder - rewinding and then recording will record over whatever is next. You can check on the authors side by taking a look in the bottom right hand corner of the recording window: If the little icon there has an I, then it's in insert mode. If it has an O, then it's in overwrite mode. Clicking will switch between the two. From the screenshot, it looks like the document is in Insert mode.
